Thank you for helping us expedite the clinic process by filling out your New Patient Forms before your visit, you may download the forms, print, fill them out, and bring them to your first visit along with:

• A list of all medications and eye drops you are currently taking, you can also bring the medications to your appointment so our technicians can take the medication names down.
• Any contact lenses or glasses you currently wear
• Your current insurance cards
• Referrals from your primary care physician if your insurance requires a referral to see a specialist.
• A valid picture identification card.
